Our program does not require this to be trained. They are instructed about having sufficient health to work in the industry. Since our students are still in training they are only informed of the requirements knowing that each employer will handle their hiring requirements differently.

Dear Amy et al -

We have procured an occupational health clinic. Students fill out a medical questionnaire which is reviewed by a physician at the clinic. Red flags are things like seizures or heart conditions or uncontrolled asthma, for which a career in remediation is not advisable. Light asthma, if it is controllable with an inhaler, is usually acceptable. Students also receive a respiratory fit test and a drug test prior to the start of class.

Hi Amy,

Memphis Bioworks Foundation’s students are required to pass a medical evaluation including a respiratory questionnaire and pulmonary function test before entering the program. We procure a service provider to provide that service, along with a drug screen at no cost to the applicant. This assessment is required by HAZWOPER and Asbestos Worker Initial training providers.

Please let us know if you have any other questions.

Thanks,

Ann Terry

Subject: Question regarding Medical Waiver

Afternoon Glo,

I have a question regarding the 40 Hour HAZWOPER Course or the grantees Job Training program in general...

Do grantees have their students sign a waiver of medical liability/assurance and/or require a physicians notes stating the student is sufficiently health to complete the course? If a waiver is signed, are there any samples that can be provided.

Thank you for your assistance,

Regards,

Amy Fanning
